Yeah, injuries happen in user v. user games even when injuries are turned off. It's yet another oversight in this game.
What's worse, and I just discovered this, is that your players can suffer multi-week injuries in user v. user games, and they stick. I just played two of my buddies in back to back weeks. I lost my starting QB in the first game for three weeks. Much to my surprise he looks like he will really be out for three weeks since he was not available in my second game. Meanwhile, one of my buddies lost his stud HB for 11 weeks in his user v. user game.
I can't confirm, but I don't think this was the case before the patch where EA "fixed" injuries. Pre-patch, players still got injured in user v. user games (with injuries turned off), but they didn't seem to start carrying over until the patch.
I actually enjoy injuries when they are realistically implemented. If HBs and QBs didn't suffer casualties at the rate of a Civil War soldier, I would just turn injuries on. But none of the guys in our league want to playing with their punter at QB after their three actual quarterbacks go down.

I really wish I had some sort of recourse against EA's unacceptable lack of quality. I stopped buying Madden after '10 when they gave absolutely no attention to the online franchise the next year. I'm done with NCAA after this year since most of their exciting new features don't work as intended. I really plan to boycott EA altogether, but the Star Wars mmo will test my resolve. Even so, it only leads to personal satisfaction when I don't support them. They won't miss my business.
Since they're all about the all-mighty dollar, they really should just make Madden and NCAA subscription based like World of Warcraft. They could make more money, but they would also have more incentive to not suck since they could lose subscribers.
